Sonic Adventure (1998)

All time: Doom second to Half Life: Opposing Force because its the only game i know of i replayed countless times with or without Mods

Recently: Super Mario 64 because having played through Sunshine after was complete torture and made me appreciate 64 way more to the point where i wanted to replay it again

Very very very very complicated question. If I was forced to say something at gunpoint though, I would say Garry's Mod. It has definitely defined a lot of internet culture, even if that influence has diminished over the years.
There is a lot of problems with it, but I think it's really a true sandbox, right next to Minecraft (which is a competitor to the"best video game ever")
